SUMMARY:

The Medical Social worker, as part of an interdisciplinary team, including the patient/family, physicians, nurses, therapists and payers, ensures that the patient’s progress in the acute episode of care through post discharge and is quality driven, while being efficient and cost effective.

The Social Worker works collaboratively with the RN Care Coordinator and interdisciplinary team to ensure patient needs are met within the context of the acute episode of care, and that care is provided within the timeframe of the acute care stay to address most critical psycho/social/financial barriers impeding progression of care during the stay, and ensures that a transition plan for ongoing management or resolution of issues is in place at the time that the patient is clinically ready to leave the cute care environment.

EDUCATION:

•Master’s Degree in Social Work Required.

EXPERIENCE:

•Two years’ experience in health care field, familiarity with medical terminology preferred

•Experience in dealing with extended care facilities and community healthcare agencies preferred

•Working knowledge of insurance coverage, various governmental programs, available community care agencies and long-term care facilities preferred

•Knowledge of Social Work Code of Ethics, Social Work theories, therapies and techniques as used in individual family and group treatment preferred

LICENSE/CERTIFICATION:

•American Heart Association “BLS” Provider Required

•Pennsylvania Social Worker Licensure preferred

•Case Management Certification (CCM) preferred
