Case Management is defined as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocating for options and services to meet an individual's health needs through communication and available resources to meet the triple aim: improve patient experience, improve the health of populations and promote cost effective outcomes.

The Social Worker is responsible for collaborating with the members of the health care team to provide comprehensive care to improve patients¿ social and behavioral concerns that impact overall wellness. Facilitate the coordination of appropriate patient centric and cost effective services that are consistent with the patient's plan of care to help achieve his/her optimal level of independence and enhance the quality of life.This position will support ambulatory sites for Penn State Health Academic Medical Group.


Master's Degree in Social Work required.


Strong communication skills both oral and written

Strong organizational skills

Ability to analyze data reports on patient outcomes


Experience as a Social Worker in an outpatient setting preferred.

LSW, LCSW certification preferred.
