As part of a dynamic interdisciplinary team, oversees inpatient care to ensure hemodynamic optimization and adherence to quality standards and core measures. Manages new referrals for Advanced Heart Failure (AHF) therapies in the inpatient and outpatient settings. Completes History and Physicals, orders diagnostic testing, and prepares patient summaries for presentation to AHF Multidisciplinary Committee. Provides primary outpatient medical management of patients with AHF therapies, addressing routine follow-up and acute concerns. Reviews outcome data and critically analyzes quality metrics. Develops quality improvement plans and coordinates implementation with the interdisciplinary team. Provides advanced education to staff members, patients and families about cardiac transplant and Ventricular Assist Devices.
Masters Degree in a Critical Care or Acute Care Nurse Practitioner program with a minimum of one year CRNP experience (which could include clinical rotations or relevant clinical experience in applicable specialty) required and three (3) years critical care experience as a staff nurse required.
Currently licensed to practice as a CRNP by Pennsylvania Board of Nurse Examiners required.
National certification as an advanced practice nurse required.
Must possess knowledge of appropriate critical care, to include advanced cardiac life support and basic cardiopulmonary life support certification.
